Patrick Lencioni has written a book - The Five Dysfunctions of a Team. In this book, he has described five elements that fail the team. The idea is to avoid these pitfalls. The five critical aspects of team building are:

Trust: Trust is the foundation of teams. Trust comes from being vulnerable. We need to be ready to accept and share our weaknesses with other team members.
Differing Views: Teams that do not trust each other cannot engage in a meaningful dialogue. They avoid conflict and focus on being politically correct. It is impossible to develop creative solutions without debate.
Commitment: People avoiding conflict compromise in making decisions. Since they do not own the decision, they do not commit to it. Things are done half-heartedly, and things rarely get executed on time.
Accountability: When teams lack commitment, they do not hold themselves or others accountable for the results.
Focus on results: Ego, recognition, and material rewards come first when there is no accountability. People pay attention to ticking the boxes instead of focusing on the outcome.
Metrics:
Assemble your team and have open discussions on the strengths and weaknesses of individual members. Go first with your vulnerabilities.
Encourage people to come up with their views and stop the bullies in the meeting to overpower them.
Define a policy that necessitates people to make decisions and execute it. Give them immunity if their actions do not yield results.
Get into the habit of preparing minutes that have clearly defined responsibilities and timelines.
State the goals and leave the authority to people for deciding the ways to reach them.
